We are using a new online service called "Register with a GP surgery" that makes it easy to register with this GP surgery.
Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number. The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register. You can still register through our website as follows. The surgery is currently accepting new patients. We require you to complete the official GMS1 purple registration form (both front and back) plus the Patient Questionnaire (see below). You can download both and print off at home if your prefer or we can post one out to you: GMS1 & New Registration Form Questionnaire

It normally takes up to 48 business hours for your application to be processed on to out system.
Before registering, We will need to see two pieces of identification from the following list:
One from this list:
- Passport
- Driving licence
- National Identity Card
- Current state pension or benefit document from DWP
- Blue Badge pass
- UK birth certificate
For under 18's living at home:
- Birth certificate
- Provisional photo-card driving licence
Also, one from this list:
- Bank, building society or credit card bill - less than 3 months old
- Mortgage statement - less than 12 months old
- Current state pension or benefits documentation
- Tenancy agreement
- Utility bill - less than 3 months old
- Local authority tax bill/council tax bill for current year
- Official letters from care or nursing home confirming residence
- Solicitors letter confirming completion of home purchase
Students
We welcome student patients at the practice. Please pop into the surgery to complete an application form.
Visitors / Temporary Residents
If you are visiting the area or are a temporary resident, we may be able to see you as a temporary resident. Please contact the surgery for further information.
If you feel that you require immediate medical attention, there is an Urgent Care centre based at the Countess of Chester Hospital.
What Happens Next
Once your application has been accepted by the practice, we will apply for your medical records from your previous surgery. Please note that this can take up to 6 weeks as per NHS guidelines.
Please ensure that you have enough medication from your previous surgery to last whilst we transition you over to our practice.
Should you need medication whilst your records are being forwarded to us, please make a routine appointment with a GP.
Please Note
We are currently not accepting registrations for patients from other surgeries based in the Fountains Health building.
At this surgery we feel that it is important to look after the entire family. Therefore we DO NOT register children under 16 yrs old without parents / guardian being registered at the practice
Practice Boundary Area
Our practice is within the West Cheshire Clinical Commissioning Group Area. We can only look after patients who live within our catchment area.
Please be aware that our catchment area does not cover patients who live in Wales as they fall under a different Clinical Commissioning Group Area.
If you move over the border to Wales you will be asked to register in that area.

To determine if your address falls within our boundary area, please click on the 'enlarge' option on the top right hand corner of the map. The map will then open in a new tab. Use the 'search' option on the top left hand corner of the map to search for your address and/or post code.
If your address falls within the boundary area then we are able to accept you as a new patient.
If you are an existing patient and have moved to an address which falls outside the boundary area then unfortunately we must ask you to register with a GP closer to your new home. This is because we can only look after patients who live within our catchment area.
As per Department of Health guidelines, we accept registrations from patients that have a permanent address in our practice area. Unfortunately we cannot accept registrations from patients who currently reside in Wales, Scotland or Northern Ireland.
PRACTICE POLICY - We are unable to register you if you live out of our practice area or over the border in to Wales. This is in line with local policy, we are not allowed to accept patients out of this area. These will get rejected by our registration links authority. We are also reviewing our list of our current out of area patients who are now out of our practice area and you may receive a letter advising you to register nearer your home address. We will give you 30 days to do this. If there is a concern, please get in touch, i.e. you are currently under referral, elderly frail/under cancer/palliative treatment. We can therefore then delay you having to move practice until appropriate.
NHS policy - The GP surgery can refuse registration for reasons such as they are not taking new patients or it's too far away from your home and you need home visits.
